Membership Details:
The dental savings plan at Dr. Anthony Martin, D.M.D offers significant discounts on dental services. To better assist our patients, the dental savings plan agreement starts on the first date of service, not by calendar year.
Monthly or annual fee for membership under benefits plan are due upon activation.
Member benefits may not be used with any other offers or insurance plans.
Agreement can be cancelled after 6 months if there is no account balance and recommended treatment is finished.
Fees for certain procedures may change throughout the lifetime of the membership.
Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, car, disability, or workman’s comp type of insurances are involved, this discount plan cannot be used.
Failure to keep appointments may result in additional fees.
